New Patients

TBSI Neurosurgery participates with a large number of insurance plans; however, you will need to verify our participation with your plan before your appointment. If you are on a managed care plan, please confirm that we have received prior authorization for your visit. When calling your insurance company to verify our participation with your plan, TBSI is best identified by giving them our Tax ID # – 741282696.

Unfortunately, we do not accept Blue Cross Blue Shield HMO, CHIP, and Star Plus. If your insurance requires a referral or authorization to see a specialist, you will be required to have a referral from your Primary Care Provider. We must have this written referral prior to scheduling an appointment.

What to bring or submit for your first appointment

As a new patient, you will be asked to bring or submit the following to your first appointment:

  • Completed New Patient Packet – It is imperative that you have completed this paperwork PRIOR to your appointment. Out of respect for all of our patients and their time, if you have not completed these forms prior to your scheduled office visit, you will be asked to either reschedule.
  • Your CURRENT insurance card (front and back)
  • Driver’s license
  • List of all current medications and dosage requirements
  • All related MRIs and CTs/studies on a disk
  • Any previous medical records, especially tests results

Your initial appointment will be scheduled with your surgeon’s specific Physician Assistant. 

The PA serves as an extension of your surgeon, and in as much the PA will initiate the first office visit to gather medical history from you, evaluate the presentation of symptoms, and begin the preliminary efforts of diagnosing your specific condition. The PA will commence the process of determining your treatment pathway, order any studies if they are needed, and will then have you come back to see the physician. Each surgeon works in tandem with their respective PAs. Collectively the PAs in our office have over 20 years of neurosurgical experience and have similar participation in national organizations supporting professional growth and continuous education.

Established Patients

Although you may have visited us in the past, we require that paperwork be updated regularly. If we have not seen you in the past twelve months, we will ask for you to complete the New Patient Packet again with updated information to provide the best care possible.

Prescription Policy

Because our providers are often caring for patients outside the clinical office either in surgery or traveling to our outreach clinics, we require up to 3 business days notice to complete refill requests for prescriptions. YOU MUST CONTACT YOUR PHARMACY to initiate the process. Refills requests are honored throughout the business week, but will not be filled over the weekend or holidays.

Please note: Due to a change in the classification of certain medications by the federal government, effective October 6, 2014, TBSI Neurosurgery clinical patients will no longer be given prescriptions for Hydrocodone (in any and all combinations). These include:

  • Norco
  • Vicodin
  • Vicoprofen
  • Lortab
  • Zydone
